Choosing the right testosterone therapy formulation…

When most people think of testosterone, they think of bodybuilding and injections. However, what many don’t realize is the vital role testosterone plays in normal bodily function in men and even in women. Testosterone is vital for maintaining muscle mass and strong bones, production of red blood cells that carry oxygen, normal sexual function, fertility, and dozens of other processes. There are many things that can impact testosterone levels including age, obesity, poor sleep quality, sedentary lifestyle, thyroid problems, diabetes, certain medications like opioids or steroids, chemotherapy, undescended testicle or testicular surgery, etc.

In men with testosterone deficiency, therapy is safe when regularly monitored. When it comes to testosterone therapy, there are over a dozen different formulations available including short acting injectables, topical ointments/creams, patches, dissolving tabs, pills, nasal spray, subcutaneous pellets, long-acting injectables. This is often surprising to many men who think testosterone therapy only involves short acting injectables. Although short acting injectables and topicals are the most commonly used, there are certain advantages and disadvantages to alternative therapies. It is very important to have an individualized treatment plan developed by your doctor.   

There are three many considerations for selecting the right testosterone formulation that I will highlight.

Specific Side Effects – All testosterone formulations share side effects related to increasing your levels. However, the side effects can occur at dramatically different percentages for different formulations such as polycythemia (elevated red blood cell counts). This is important if you and/or your doctor want to avoid specific adverse effects associated with testosterone therapy, but still be able to increase your levels.   

Ease of Administration – Some men aren’t excited about repeatedly injecting themselves. Some find that ointments and creams are messy and inexact. Some have a difficult time remembering multiple doses of therapy in a given day. Based on your specific preferences, one formulation may be better than next.

Cost  - In many cases, testosterone therapy and routine monitoring can be covered by insurance. This is surprising for men who may have had a prior experience at a wellness or lifestyle clinic, where they were asked to fork over $200-500 a month for monitoring, labs, and the medication. However, for the newer, branded formulations, many insurances will ask that you go through their formulary first or demonstrate appropriate indication prior to initiation.
